Fault tree can be useful in safety analysis. Fault tree analysis starts with making a list of faults in a system and finding it back to its root cause. Let us take the case of a forklift accident. Broken tynes, broken wheel hangers, or failing brakes can all be traced to failure to complete a inspection list. These problems can be detected through the inspection list. The checklist is the operator’s responsibility and the operator is the supervisor’s responsibility. The event tree diagram can work the opposite way. List events that could lead to faults if not done. Going back to our example an event listed could be the supervisor not doing his job of checking on the operator. This leads to the operator not doing his job on the inspection list. This leads to catastrophe, because the hangers, tynes, and brake will fail.
